The talon.by service has been available via the Internet since 2010. Today, with its help, you can order a coupon to see a doctor in 49 Belarusian clinics, two more clinics will join the system in the near future. The website also allows you to call a doctor at home. 39 of the 49 available healthcare institutions are located in Minsk, but the system developers plan to connect the majority of Belarusian clinics by 2015.

About 50 thousand coupons are ordered on the site per month, and the number of unique visitors reaches 70 thousand. In total, more than half a million coupons were ordered during the existence of the site, and over the past year the number of orders has increased 1.5 times.

For developments in the field of healthcare informatization, JSC Mapsoft was awarded the TIBO-2013 Internet award in the field of medicine.


How the services work

In May 2013, the talon.by website was seriously updated. Before this, the developers placed the main emphasis on functionality, but now the site has acquired a friendly interface. In addition, a separate mobile version was developed. According to Kirill Polyukhovich, a leading specialist at Mapsoft, it is no longer necessary to create a separate application for smartphones - all the functionality of the site is available through the mobile version.

Many changes were made taking into account user feedback. Currently, the site’s technical support receives only about three requests a week from users, most of them concern not the functions of the site, but the work of the clinics. The help section of the site is almost entirely composed of frequently asked questions, divided into sections.

According to Polyukhovich, the Mapsoft system has competitors, but most of them have a significant drawback: they are not directly connected to clinic databases. This means that clinics need to manually track coupon orders and allocate coupons for online ordering separately from regular coupons.

All available coupons to doctors are immediately available on the website talon.by; clinics themselves choose a quota of coupons that can be ordered through the website. This quota is used on average by 75%, and in some specialties by 100%, but these same coupons remain available for order through the registry. The site is directly connected to the clinic’s database: as soon as the user orders a coupon, the order is taken into account automatically without the participation of the registry. And if a doctor is called to your home, notifications are immediately sent to the reception desk.

Unfortunately, one printout of a coupon ordered via the Internet is not enough in most clinics - you need to confirm the coupon at the reception desk. According to Polyukhovich, this is due to the policy of clinics who want to receive confirmation that the patient came at the appointed time.

You can order a coupon either for a specific time (to a doctor who is free at that time) or to a specific doctor (by choosing a time when this doctor is free). To order a coupon, you do not need to register on the website (although the developers plan to add optional registration for regular users) - just enter your last name, residential address and date of birth. After this, the user becomes available to the user account, in which you can also view already ordered coupons.

In addition to online ordering of coupons, Mapsoft also offers clinics the installation of information kiosks. They duplicate the functions of the site on site, but the coupon received at the information kiosk no longer needs to be confirmed at the reception. In total, 20 information kiosks have been installed in Belarusian clinics.


Opinion of the head physician

One of the first Belarusian clinics to take the path of computerization was the 34th clinic. According to the head physician Natalya Dergach, there are now more than 200 computers installed in the clinic. Clinic employees can no longer imagine their work without computers.

Since last year, the clinic has had its own call center with the support of Mapsoft. Patients no longer need to call the reception desk; they just need to wait for an available operator. The call center also allows you to automatically notify patients about the need to undergo a procedure (for example, if the patient has not undergone fluorography for a long time). The average wait time is 3 minutes, and of the 787 calls received by the call center on August 19, the longest wait on the line was 7 minutes.

Such statistics, as well as information about ordered coupons, are available to the head physician in real time. According to Natalya, this makes the head physician’s work easier; instead of paperwork, time can be spent working with people. The same applies to the work of the registry, which online registration eliminates some of the routine paperwork.

Dergach also said that, despite fears, there were almost no false orders of coupons or hooligan attacks on the system. The Mapsoft system allows the registry to control orders of coupons and weed out “garbage” ones both automatically and manually. In addition, the user can only order 5 coupons at a time. “We were going to introduce a system for blocking unscrupulous users, but it turned out to be simply not necessary”, added Polyukhovich.

According to Natalya, the website talon.by did not add any extra work to the registry; on the contrary, it reduced the load: a significant part of booking coupons occurs simply without the participation of clinic employees. True, the registry staff has been reduced slightly: outpatient cards remain “on paper”, and registry employees are required to personally deliver them to the offices.

Natalia Dergach also said that electronic services do not pay off directly (or will pay off in the future due to a reduction in reception staff), but pay off indirectly through more efficient work and rational use of working time.


Future plans

The plans of the developers of the online registry include not only the predictable expansion of the system to all clinics of the republic and the widespread installation of information kiosks. Mapsoft plans to expand the functionality of the online registry, connect institutions without their own databases, and link the accounts of relatives.

The developers also want to add the ability to order coupons based on a doctor’s referral in “foreign” healthcare institutions, and not just in the user’s “native” clinic. However, this can only be done through the “native” registry due to the problem of identifying the patient in a “foreign” institution. However, you won’t have to go further than your “native” registry to get the coupon.

The company is technically ready to implement electronic outpatient cards and medical records, but this is hampered by organizational and financial issues. Legislation does not allow us to completely eliminate paper document flow in healthcare (despite the possibilities of digital signatures). The company plans to begin an experiment to implement an electronic patient record in 2015.

Mapsoft is also developing an electronic queue system and plans to replace information stands in clinics with electronic displays. The electronic queue will reduce the waiting time for an appointment and streamline the order of visiting a doctor. With its help, you can also resolve disputes between patients with coupons and urgent cases - the system will allow you to calculate the required time for an appointment and the availability of available doctors.


So far, the main problem of the electronic queue is precisely the time standards. Organizing a queue at a clinic is much more difficult than, for example, at a bank: patients do not care which office they go to; urgent referrals, reserved coupons, and redirections from one doctor to another must be combined. But the developers do not consider these problems insoluble and promise experimental electronic queues in clinics in the near future.

Registration on the site provides the user with access to a personal account in which the following functions are available:
- saving patients’ personal data and the ability to quickly switch between them, which significantly speeds up the processes of ordering coupons or calling a doctor;
- viewing and/or cancellation of ordered coupons;
- automatic notifications to the specified e-mail about ordering a coupon and reminders on the eve of an appointment with a doctor.
Our service is constantly evolving, so in the future the list of available functions will expand significantly.
In addition to the obvious advantages, registration makes it possible to identify the user, while ensuring a high level of security of personal data. Thanks to user identification, the administration of each medical institution can influence patients who order a large number of coupons but do not show up for an appointment with a doctor. Increasing the responsibility of patients contributes to an increase in the quota for issuing Internet coupons, which makes making an appointment with a doctor via the Internet accessible to a larger number of patients. TUT.BY correspondents spent the morning of Wednesday, February 1, in three Minsk clinics - counting how many patients one therapist manages to see, how many Minsk residents come into the office “just to ask,” and how many at the time using coupons. Surprisingly, there was no queue at any of the clinics.

Clinic No. 6: 12 people in two hours

From 8.00 to 10.00, the corridors of one of the therapeutic departments of clinic No. 6 are not crowded. We choose room 411 for observations. It’s already past nine, and there’s only one person waiting in front of the door with a ticket. Looks like there's a first visitor to the doctor's office right now.

By nine o'clock, according to our calculations, 9 people have time to visit the therapist's office. One of them is a gray-haired man without a ticket - he needs a direction.

A new hour begins. The silence is broken by a breathless young man:

“I’m just a little late,” he says apologetically. - 9.02, and the ticket is for 9.00.

However, the patient has nothing to apologize for and, in fact, no one to: no one takes his time for a few more minutes. A guy walked into the therapist’s office at 9.07. He will be with the doctor until 9.25.

At 9.22, an elderly woman wearing a headscarf came to the therapist’s office. A man had already lined up in front of the door before her. There is some kind of dialogue between the patients.

— What is your coupon number? - says the patient.

“Sixth,” the patient answers, without looking up from his mobile phone.

- And I have the seventh. That's how well it turned out. — He sits down and tries to attract the attention of his neighbor in turn. “Look what good photocopies they made for me.”

“The seals are different,” the man reacts, apparently again without raising his eyes.

The woman sits silently for a couple of minutes, and then goes into one of the offices nearby and says to talk to the manager she knows.

At approximately 9.25 a man with coupon No. 6 enters the office. There it stays for about 20 minutes. A woman who came out of one of the offices had already taken off her headscarf and sighed:

- Probably missed my turn...

When she finds out that she didn’t miss it, she begins to sigh for another reason:

- Oh, how slow! I have a ticket for 9.30, but it’s already 9.45 and I can’t get in.

At 9.50 the patient enters the office. Another elderly woman takes her place. She arrived ahead of time - she only had a ticket for 10.15.

According to our calculations, 12 people managed to visit the therapist’s office before 10.00. Almost everyone came with coupons. Two of them came in “just to ask.” And they kept their promise: they left in a minute at most.

Clinic No. 2: “Many people order coupons on the Internet and don’t show up”

We start the morning at clinic No. 2 on Yakubovsky 33 with the registration desk. At 8:00 a.m., a man in front is interested in a voucher to see a therapist.

— Is it okay for 9.40? — the girl in the window clarifies.

The patient agrees. Meanwhile, in one of the rooms of the therapeutic department, the appointment has already begun.

At 8.10 there are two women in line, with coupons No. 2 and No. 4.

— Are there those who don’t have coupons?

The patients look at each other with displeasure. They remain silent and with a menacing look lead the journalist to the nearest bench: there is no point in disturbing the order, so to speak.

-Who is the last one? - a pensioner approaches. - I'm fifth, I'll be behind you.

Sits next to women. Silence.

8.14 - the patient comes in. 8.26 - she comes out. The next one is behind her.

- I'm in acute pain! — a thick-set man of about 40 booms.

And at the same time, an appointment with a therapist next door begins.

— How much do you have a coupon for? - the pensioner asks the girl.

- At nine.

- Oh, I have ten. But you know... You'll miss me, I'll quickly... Just a couple of recipes.

- Faster for me. I'll just measure the pressure. Maybe the man behind me will let you through.

The man is silent.

The clock is ticking, the good-natured pensioner leaves the doctor at 9.27. The fact that it was taken for so long, in general, does not particularly outrage anyone. There was no one in the queue except the journalist.

Closer to 9.30 a girl came up and went straight to the office.

The coupon, he says a little later, was ordered online last week.

In principle, this could have been done yesterday, but it would hardly have been possible to find a convenient time.

“I don’t even understand what kind of silence this is in the clinic.” Just like it’s not winter outside! Isn’t anyone sick here anymore? — the patient of the neighboring therapist keeps up the conversation. — I’ve been going to the therapist for two weeks, and there are no queues. Cards, however, are poorly carried. I would have already gone to my doctor, but the cards from the registry had not yet been delivered.

“They wanted everything at once,” another patient counters. — There are few people, they wait until the pile is collected and carry it. And you're being nitpicky here.

“I’m not finding fault, I’m catching illnesses here at this time,” the second one does not concede. — The other day I got stuck in the elevator with one, because she started up: cough, cough. At least she'd cover her mouth. I couldn’t get up for three days because of the cough.

The patients of the neighboring therapist quickly agreed on the topic of sores, and meanwhile the clock was 9.40. There is no one under the office.

9.45, 9.50 - no one.

9.53 - a woman without a ticket stopped by. At ten o'clock the second one came to close the sick leave.

We go down to the registration desk. The next coupon is offered only for Friday.

- How so? There's literally no one there under the two doctors' offices.

“Yes, you go like that,” the receptionist instructs. — Many people now order tickets via the Internet, and then don’t cancel and don’t show up. So it turns out: no coupons, no patients.

Clinic No. 30: “There are few people, it’s even strange”

At 7.50, two people are already waiting at the therapist’s office - a guy and an elderly woman. Soon several people arrive.

-Who's first? - asks one of the women who came.

“I have my first ticket,” says another.

Here a guy enters the conversation. He says he doesn’t have a ticket, but he’ll still go first because he arrived at 7.30. He says he hasn’t been able to see a therapist for the 4th day.

The reception began at 8.05. During the 10 minutes that the guy spent with the doctor, only one woman came - with a coupon.

The time allotted to each patient is 7 minutes. But in reality, some of the patients sat in room 5, some for 10, and some for 20 minutes.

There was no big gap between the time on the coupon and the real one, because some patients simply did not show up for their appointment.

“There are few people today, it’s even strange,” summarizes one of the grandmothers while waiting for their turn. Others are joining in as well. Women conclude that “people just haven’t woken up yet.”

One cannot but agree with them: from 8.00 to 10.30 three people came “just to ask” and 15 came to see a doctor using a coupon.

“I can’t say that patients will completely avoid queues at clinics”

“We are also monitoring the situation and going to healthcare institutions. And for us, when large queues form, this is already a rarity, a non-standard situation,” says Dmitry Cherednichenko, First Deputy Chairman of the Health Committee of the Minsk City Executive Committee.

According to the specialist, online registration and pre-registration at the registry helped solve the age-old problem of all clinics.

“Of course, I can’t say that patients will completely avoid queues at the clinic,” the interlocutor noted. “But everything we do helps to properly plan the work of healthcare institutions.

— At the same time, online registration has given rise to another non-standard situation: the doctor has no patients, but you can’t get a ticket to see him, since someone signed up and didn’t show up.

— Currently, the percentage of non-income from online registration is plus or minus 10−15%. For this reason, we do not post all coupons on the Internet, but 20-30%. Maximum 40% - to individual specialists. Let me note that our reception has a connection with any office and must immediately respond to such a situation. If necessary, specialists refer the patient to a free doctor. In addition, we always have an administrative duty officer, a therapist on duty, and a medical assistant.

Dmitry Cherednichenko said that they did not consider the issue of punishing those patients who, having booked a ticket via the Internet, did not come.

“Here we can only rely on the person’s conscience; if you signed up, then come over.”
